Apply For A Companion Dog

Thank you for your interest in adopting a companion dog from the Sgt. BAM Foundation! Our program is open to any U.S. Veteran who lives in the greater St. Louis, MO area and in the State of Missouri (within 30 miles of our office in Creve Coeur, Missouri) and is able to take care of a dog and could benefit from having a dog.

Steps to apply for a companion dog

If you are interested, you must:

  • Complete the application form.
  • Provide 3 references.
  • Provide a letter of recommendation or approval from your health care provider.
  • Provide a copy of your DD-214.
  • Agree to telephone and in-person interviews and home visits so we can know you and your needs better and discuss the information in your application.
  • Provide veterinary information if you’ve previously had a dog.

You must also meet the following requirements to be considered:

  • You have a physical and/or cognitive disability that can be impacted by an companion dog.
  • You served the United States in the Army, Navy, Air Force, Marines, or Coast Guard and received an honorable or general discharge.
  • You are released from all service and reserve duties.
  • You must be willing to participate in 120 hours of training with our training specialist and companion dog. Training will take place at the training facility in St. Peters, Missouri. Training will also take place in your home and include places you frequent such as your workplace, grocery store and medical appointments.
  • You are required to participate in follow-up contact, which may include phone calls, emails, questionnaires and visits by Sgt BAM Foundation volunteers. If required, you’ll need to attend additional training sessions throughout the partnership.
  • You must have stability in your home life.
  • You must agree to no new pets during the application process and for at least one year after placement.
  • You must be able to physically, financially and emotionally meet the needs of your service dog. If other individuals will assist with the dog’s care, they must be dependable and available for training sessions.
  • You must provide the dog with ample opportunity to perform the skills it has been trained to do.
